The fee for the treatment will be deemed due accordingly. The term treatment is used for sessions of all therapies detailed on this Website.

PAYMENT

Treatments, herbal formulas, and retail items are to be paid for at time of service/receipt of product. For those patients who choose reimbursement from insurance via Superbill, full payment is due at time of service at each appointment. Patients are responsible for ensuring they have acupuncture benefits and submitting Superbills to their insurance company. Roots to Blooms Acupuncture & Wellness, LLC does not submit any insurance documents on any patient’s behalf. There are no refunds on treatments, herbal formulas, and retail items.

APPOINTMENTS & CANCELLATION

Existing patient appointments can be booked in the clinic, before or after your treatment, or online via the website rootstobloomsacupuncture.com (link needed). Please arrive 5-10 minutes early for your appointment so the clinic can run smoothly. This also allows for a few minutes of time to use the restroom, enjoy a cup of tea, and relax before your appointment begins.

Roots to Blooms Acupuncture & Wellness, LLC requires 24 hours notice for cancellation or rescheduling of appointments. We reserve the right to bill you the full cost of your session for any appointment that is cancelled or rescheduled without 24 hours notice. Those who continually reschedule, cancel , or arrive late to appointments or disrupt the flow of the clinic may be dismissed from care at Roots to Blooms Acupuncture & Wellness, LLC.

DISCLAIMER

Health and wellbeing are the responsibility of the individual. Advice and treatments provided by Roots to Blooms Acupuncture & Wellness, LLC are complementary and not an alternative to conventional medical opinion or treatment.
